The final options are surgery vs ‘do nothing’. Onychomycosis (fungal nail infection) often doesn’t need a treatment, unless you’re immunocompromised it’s just a cosmetic thing. Worked at a podiatry clinic for 3 years and we recommended tolcylen a lot. I’ve seen great results on patients who were very compliant with applications and didn’t have nails that were too far gone (think thicker than they are long). It’s important to note that you aren’t looking for any changes to the existing nail, just the new growth from the base of the nail. If you’re seeing at least 1mm of clear growth a month you’re good, do not stop until the entire nail has grown out healthy.
